For example you started a quiz, but don't complete it (close a browser, navigate to another page and etc.). In this case complete the quiz session and start a new one to see "name" and "email" fields are enabled.

The quiz (by the link which you sent) are loaded fine for us in Google Chrome and FireFox browsers and it loads without quiz description page.

Could you explain how to reproduce the problem?

This is the link and the access button is not displayed to quiz


I want every user accessing the web enter your name and email to subsequently send the results.

Check value of "Access and Security -> Access" parameter in quiz settings. If you want to grant access for guest users, set it to "Public" for example.
