Set "Access and Security -> Anonymous access" parameter to "No" or "By user's wish" value in quiz settings.

If you want to send a mail at the end of a quiz to an administrator, enter administrator email in "Results settings -> Notification email(s)" parameter and choose a mail template in "Results settings -> Template" parameter.

Select mail templates in "Results settings -> Choose template type -> Mail successful template" and "Results settings -> Choose template type -> Mail failed template" drop-downs in quiz settings. If you want to send mail automatically, set also "Results settings -> Auto send mail" parameter to "Yes".

I've done everything you specified in Mozilla Firefox and not allowed to enter Name and Email and Google Chrome if allowed.

An access button is not displayed.
