I may have an idea what's going on. It has to do with how I am enqueuing jquery. I am going to do some experiments today and let you know.

Alas, that didn't have any effect. Still not sure why this isn't working. Did you get my email with the information to go in and look?

We found the problem. The hook doesn't work if quiz lead form is not integrated with e-mail service (like MailChimp, MailerLite and etc.). We will extend this functionality in the next version of the plugin.

Update the plugin to v. 1.5.29 please. It calls the hook even if integration with e-mail marketing services is not used.
