Hy,
how can I have a randomized list of questions (50) from 5 different question categories (10 each) that contain more that 500 questions.

Hello,

Enable "Random Question" parameter in quiz settings and "Question Count" parameter in question categories settings to "10". The component will get 10 random questions from each question category.

What about if I have 3 categories active at the moment and the quiz just shows 20 questions from only 2 categories?
Where I am wrong?

Check that "Question Count" parameter in quiz settings is set to "0". If you haven't finished quiz session which you started when only two categories were active, new settings will be applied when you finished the previous quiz session. If it doesn't help, could you provide temporary access to your Joomla! backend by email that we can investigate this problem?
