How can I add more categories to a quiz?
I added four different bank categories, but should they be quiz categories?
Sorry, I'm clearly very confused by what should be where?!

It is possible to create as many question categories as you want. Click by "New" button on quiz question categories page to create a question category.
Question bank is used if you want to use the same questions in different quizzes otherwise a quiz can be populated with questions directly.

Its almost there now, thank you for the help.
The only issue is the order of categories, is there any way to control that?
Eg.
I'd like to see the questions from category 1, then cat 2, cat 3, cat 4, at the moment its also randomising which category appears.

Sorry, but currently the extension doesn't support ability to order of question categories.
