One for all quizzes or different checkbox for each quiz? You can place it on quiz description place. See quiz.html.php template file.

<joomla_dir>/components/com_ariquizlite/view/quiz.html.php. Regarding the second question, what you mean you want to create link to specific quiz?

Link to specific quiz looks like:

index.php?option=com_ariquizlite&task=quiz&quizId=<quiz_id>

<quiz_id> is ID of necessary quiz.
