Hello,

Open module settings page (via "Extensions -> Modules" page on Joomla! backend), set "Choose source type" parameter to "CSV" and populate "Choose source type -> CSV -> Path to CSV file" parameter with path to the CSV file. Data from this file will be shown by the extension. For example if want to show data from <joomla_directory>/images/report/myreport.csv file, populate the parameter with images/report/myreport.csv value. Files can be uploaded via Joomla! media manager, a file manager extension like eXtplorer or by FTP. Other module parameters can have default values.

Check also common module parameters (which are the same for all other Joomla! modules). Open "Menu assignment" tab and check that it is assigned to correct site pages. Check also that the module is enabled.
